Mirtazapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
Although in vitro research and a case report suggest that kava inhibits CYP1A2, more robust clinical evidence shows that kava has no effect on CYP1A2. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP1A2 activity.

Here's the honest picture for kava and mirtazapine (Remeron). On paper, kava was thought to slow down a liver enzyme called CYP1A2, which helps break mirtazapine down. If that were true, your body could clear the drug more slowly and you'd notice stronger effects like extra drowsiness, grogginess, or a bigger appetite. But the better-quality research actually shows kava has little or no real effect on this enzyme, so this particular interaction is unlikely to cause trouble.

My bigger concern is overlap, not metabolism. Both kava and mirtazapine can make you sleepy and relaxed, so together they may leave you more drowsy than expected. Kava can also rarely stress the liver. Please run this by your pharmacist or doctor before combining them.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the short version on combining kava with mirtazapine. In a test tube, kava looked like it might slow down CYP2D6, one of the liver enzymes that helps clear mirtazapine. On paper, that could let mirtazapine build up a bit, which might mean more drowsiness, dizziness, increased appetite, or that heavy, sedated feeling. But when this was actually studied in people, kava did not meaningfully change how the body handles CYP2D6 drugs, so this concern stays mostly theoretical.

The bigger thing I'd flag isn't the enzyme at all. Both kava and mirtazapine can make you sleepy, so together they may leave you groggier than expected. Kava has also been tied to rare liver injury, so please loop in your pharmacist or doctor before pairing them.

Here's the honest picture for kava and mirtazapine. On paper, kava was thought to slow down CYP3A4, one of the liver enzymes that helps clear mirtazapine. If that were true, mirtazapine could build up a bit and you might feel extra drowsy. But the better-quality human study showed kava had no real effect on this enzyme, so this particular concern is mostly theoretical and unlikely to cause problems.

That said, I'd still be cautious. Both mirtazapine and kava are calming, so stacking them could leave you more sedated, sleepy, or groggy than expected. Kava has also been tied to rare liver injury. Please check with your pharmacist or doctor before combining them, and don't change anything on your own.

What Is Mirtazapine Used For?

Brand name: Remeron

Mirtazapine tablets, mirtazapine orally disintegrating tablets, Remeron, and RemeronsолTab are all indicated for the treatment of major depressive disorder (MDD) in adults. Major depressive disorder involves a persistently low or sad mood most days, often accompanied by loss of interest, sleep changes, fatigue, and difficulty concentrating.

The effectiveness of mirtazapine tablets in maintaining a response for up to 40 weeks after an initial treatment period has also been demonstrated. Patients should be reassessed periodically to determine whether continued treatment is needed.
