Combining kava with CNS depressants can have additive sedative effects.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.

The piece of this combination product that matters here is the codeine. Codeine is a narcotic pain reliever that already slows down your brain and nervous system. Kava has its own calming, sedative-like effects on the brain. Put the two together and those effects can stack up, leaving you more drowsy, foggy, dizzy, or slow to react than you would expect from the codeine alone. In stronger cases, breathing and reflexes can be dampened too.

This is considered a notable interaction backed by good evidence, so I would not pair them on your own. There's also a separate concern: both kava and the aspirin in this product can stress the liver. Please check with your pharmacist or doctor before combining them, and don't drive until you know how you feel.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

The piece of this combo product to focus on is the codeine. Codeine is what we call a prodrug, meaning your body has to switch it on using a liver enzyme called CYP2D6 before it can relieve pain. In a test tube, kava looked like it might block that enzyme, which in theory could leave you with less pain relief from the codeine. But here is the reassuring part: actual human studies show kava does not meaningfully change how this enzyme works in real people, so this is mostly a paper concern.

The aspirin and the antacids (aluminum and magnesium) are not affected by this pathway. My bigger caution with kava is its rare link to liver damage, and since aspirin can also stress the body. Please run this by your pharmacist before combining them.
