P-GLYCOPROTEIN SUBSTRATES

Ombitasvir, Paritaprevir Ritonavir falls under P-GLYCOPROTEIN SUBSTRATES — the group Kava interacts with.
In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with your Technivie (ombitasvir, paritaprevir, and ritonavir) and kava. This combo is partly handled by a transporter called P-glycoprotein, which acts like a pump moving the drug around your body. In lab dishes, kava seemed to slow that pump, which in theory could let a bit more medication build up. But the real-world evidence is weak and mixed. A human study using another pump-sensitive drug (digoxin) showed kava didn't really change its levels, so this concern is mostly theoretical.
The bigger issue is your liver. You're treating hepatitis C, and kava has been linked to rare but serious liver injury. Mixing it with a liver-focused medication isn't wise. Please check with your pharmacist or doctor before using kava.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Ombitasvir, Paritaprevir Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the gist for Technivie (ombitasvir, paritaprevir, and ritonavir): part of this combo, including the paritaprevir, is handled by a liver enzyme called CYP3A4. In a test tube, kava looked like it might slow that enzyme down, which in theory could let drug levels climb. But when researchers actually gave healthy people kava for 28 days, it had no real effect on CYP3A4. So this concern stays mostly on paper.
My bigger worry is different. Kava has been tied to rare but serious liver injury, and you'd be taking it while treating hepatitis C, which already stresses your liver. That's the real reason to pause. Please talk with your pharmacist or doctor before combining them.

The combination of ombitasvir, paritaprevir, and ritonavir is usually used in combination with ribavirin (Copegus, Rebetol), but sometimes it is used alone to treat a certain type of chronic (long-term) hepatitis C infection (swelling of the liver caused by a virus). Ombitasvir is a hepatitis C virus (HCV) NS5A inhibitor. It works by stopping the virus that causes hepatitis C from spreading inside the body. Paritaprevir is a protease inhibitor. It works by decreasing the amount of HCV in the body. Ritonavir is a protease inhibitor. It helps to increase the amount of paritaprevir in the body so that the medication will have a greater effect. It is not known if ombitasvir, paritaprevir, or ritonavir prevent the spread of hepatitis C to other people.
