Why this applies to Sertraline

Sertraline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the good news about pairing kava with your sertraline (Zoloft): on the metabolism side, this one looks pretty quiet. Early lab tests suggested kava might block CYP3A4, an enzyme that helps clear sertraline, which in theory could let levels creep up. But a stronger study in real people taking kava for 28 days showed no meaningful effect on that enzyme. So the worry about kava boosting sertraline and causing more nausea, drowsiness, or jitteriness is mostly theoretical and unlikely.

My bigger caution isn't about enzymes at all. Both kava and sertraline can affect mood and add to drowsiness, and kava has been tied to rare liver injury. Please check with your pharmacist or doctor before combining them.

Kava is a Pacific Island plant traditionally used to promote relaxation and ease anxiety, and some studies suggest it may help mild anxiety. However, kava has been linked to rare but serious liver damage, so it should be used with caution, avoided by people with liver problems, and only used after talking with your pharmacist or doctor.

About Sertraline

What Is Sertraline Used For?

Brand name: Zoloft

Sertraline treats a range of mental health conditions. All formulations are approved for major depressive disorder (MDD) in adults and obsessive-compulsive disorder (OCD). The oral solution (including Zoloft oral solution) and several tablet formulations are also approved for panic disorder, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD).

For OCD specifically, the capsule formulations and some tablet products are approved for use in adults and children aged 6 and older. Sertraline hydrochloride tablets (plain, not film-coated) are approved for OCD in pediatric patients as well. However, sertraline is not approved for major depressive disorder in children — only in adults.
