CNS DEPRESSANTS

Atropine, morphine sulfate falls under CNS DEPRESSANTS — the group Kava interacts with.
Kava has CNS depressant effects. Concomitant use of kava with other CNS depressants can increase the risk of drowsiness and motor reflex depression. Clinical practice guidelines from a joint taskforce of the World Federation of Societies of Biological Psychiatry (WFSBP) and the Canadian Network for Mood and Anxiety Treatments (CANMAT) recommend that CNS depressants, including alcohol and benzodiazepines, not be used with kava.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
The piece to focus on here is the morphine. Morphine is a strong opioid pain reliever that already slows down your brain and breathing. Kava has its own calming, sedative effect on the nervous system. Put them together and those effects stack up, so you could feel much more drowsy, foggy, dizzy, or unsteady than morphine alone would cause. The bigger worry is slowed, shallow breathing, which can become dangerous.
This isn't about one product speeding up or blocking the other. It's simply two sedating things added together, and expert guidelines specifically warn against pairing kava with this kind of medicine. Please don't change anything on your own. Talk with your pharmacist or doctor before using kava while you're on morphine.

P-GLYCOPROTEIN SUBSTRATES

In vitro research shows that kava can inhibit P-gp efflux. However, a clinical study in healthy volunteers shows that taking kava standardized to provide 225 mg kavalactones daily for 14 days does not affect the pharmacokinetics of digoxin, a P-gp substrate. It is possible that the use of other P-gp substrates or higher doses of kava might still inhibit P-gp.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product pairs morphine (a strong opioid pain reliever) with atropine. Morphine is carried around the body partly by a transporter called P-glycoprotein, which acts like a bouncer moving the drug out of cells. On paper, kava might slow that bouncer down, which could let a bit more morphine linger in your system. If that happened, you might notice more of morphine's usual effects, like extra drowsiness, slowed breathing, constipation, or feeling lightheaded.
Here's the honest part: the evidence for this is weak. It comes mostly from lab dishes, and a human study using kava showed no real effect on a similar transporter drug (digoxin). So this is more theoretical than proven. Still, since both kava and morphine can make you sleepy and both can stress the liver, please run this past your pharmacist or doctor before combining them.

CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
3 of 3Atropine, morphine sulfate also falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the practical scoop on combining kava with this morphine and atropine product. In a test tube, kava looked like it could block CYP2D6, one of the liver's drug-processing enzymes. If that actually happened in your body, drugs cleared by that enzyme could build up. But when researchers tested this in real people, kava did not meaningfully change how CYP2D6 medications were handled, so this particular concern stays mostly theoretical.
What I'd watch more closely is overlap in effects. Kava can make you drowsy and relaxed, and morphine already causes sleepiness, slowed breathing, and constipation. Stacking two calming things can add up. Kava can also stress the liver. Please check with your pharmacist or doctor before pairing them, and never adjust your morphine on your own.
