Lopinavir, Ritonavir falls under HEPATOTOXIC DRUGS — the group Kava interacts with.

Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and Kaletra (lopinavir/ritonavir). This isn't about kava changing how the drug works in your blood. Instead, both can be tough on your liver. Kaletra, like several HIV medicines, can sometimes raise liver enzymes or stress the liver, and kava has been tied to rare but real cases of liver injury, mostly with heavy or long-term use.

Stacking two things that lean on the same organ may add up. Watch for signs like yellowing skin or eyes, dark urine, belly pain, unusual tiredness, or nausea. To be honest, this is theoretical and based on scattered case reports, not strong proof. Still, please check with your pharmacist or doctor before combining them, especially since your HIV therapy already needs liver monitoring.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Lopinavir, Ritonavir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the honest picture with Kaletra (lopinavir/ritonavir) and kava. The thinking was that kava might slow down CYP3A4, the liver pathway that handles this HIV medicine. If that happened, your levels could creep up and you might see more side effects like nausea, diarrhea, or stomach upset. But the better-quality human research doesn't back that up. In a study where people took a solid dose of kava daily for a month, CYP3A4 activity didn't budge.

So this interaction is rated minor and unlikely. My bigger concern is your liver: kava has been tied to rare liver injury, and HIV meds can stress the liver too. Please check with your pharmacist or doctor before combining them.
