Niacin, Lovastatin falls under HEPATOTOXIC DRUGS — the group Kava interacts with.

Theoretically, using kava with hepatotoxic drugs might increase the risk of liver damage.
Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the thing to keep an eye on with Advicor: it combines niacin and lovastatin, and both of those can be hard on the liver on their own. Lovastatin (a statin) and high-dose niacin both occasionally raise liver enzymes. Kava has its own reputation for rare but serious liver injury, so the worry is simply stacking two liver stressors together.

This concern is theoretical and based on scattered case reports, not strong studies, so I don't want to scare you. But it's a reasonable caution. Watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual fatigue. Before pairing these, please check with your pharmacist or doctor, who may want to keep an eye on your liver bloodwork.

Niacin, Lovastatin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with Advicor (the niacin and lovastatin combo): the worry on paper is that kava might slow down an enzyme called CYP3A4, which is the same enzyme your body uses to clear the lovastatin part of this medicine. If that enzyme were blocked, lovastatin could build up and raise the small chance of muscle aches or, rarely, more serious muscle problems.

But here's the reassuring part. While early lab dish studies hinted at this, a better human study found kava did not actually affect CYP3A4. So this interaction is rated minor and unlikely. My bigger concern is that both kava and statins can stress the liver, so it's worth a quick chat with your pharmacist or doctor before using both.
