Buproprion, Dextromethorphan falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Kava interacts with.

It is unclear if kava inhibits CYP1A2; research is conflicting.
In vitro research shows that kava extract significantly inhibits CYP2D6. However, clinical research shows that kava does not affect the metabolism of CYP2D6 substrates in humans.

Here's the gist with Auvelity (the bupropion and dextromethorphan combo). The dextromethorphan part is cleared by a liver enzyme called CYP2D6. In a test tube, kava looked like it could block that enzyme, which in theory could let dextromethorphan build up and add to side effects like dizziness, drowsiness, or nausea. But that's only the lab picture.

The reassuring news: when researchers actually tested this in people, kava did not meaningfully change how CYP2D6 drugs are handled. So this is rated minor and unlikely. I'd still mention kava to your doctor, since both kava and bupropion can stress the liver, and that's the bigger reason to be careful here.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Auvelity combines dextromethorphan with bupropion, and that pairing is what makes this combo interesting. The bupropion is there partly to slow down how fast your body clears the dextromethorphan, helping it reach the brain levels needed to lift mood. On paper, kava was flagged as a possible blocker of CYP3A4, one of the enzymes that handles dextromethorphan. If that were true, levels could nudge upward, with effects like dizziness, drowsiness, or nausea.

Here's the reassuring part: better-quality human research showed kava did not meaningfully affect CYP3A4. So this interaction is mostly theoretical and unlikely to cause trouble. The bigger concern with kava is rare liver injury, especially since bupropion also affects the brain. Please run it by your pharmacist or doctor first.
