Theoretically, berberine might increase serum levels of drugs metabolized by CYP2D6.

Here's the simple version: your body relies on a liver enzyme called CYP2D6 to clear dextromethorphan (the cough suppressant in Vicks Formula 44). Berberine appears to slow that same enzyme down. So when you take them together, dextromethorphan may hang around longer and build up a bit, which can nudge its effects and side effects upward. Think drowsiness, dizziness, a foggy or lightheaded feeling, or stomach upset.

I want to be honest about the strength of this. It's mostly lab work plus some early human data, so it's more of a theoretical, possible concern than a sure thing. For an occasional dose of cough syrup, it's usually low risk, but if you take berberine daily or use dextromethorphan often, check in with your pharmacist or doctor first.

Here's the gist for your cough medicine. Berberine can gently slow down one of the liver enzymes (CYP3A4) that helps clear dextromethorphan from your body. When that happens, a little more of the drug can hang around, which may mean stronger or longer-lasting effects like drowsiness, dizziness, or an upset stomach.

The good news: this is mostly a theoretical concern, and dextromethorphan is usually taken for just a few days when you have a cough. For most people, a short course alongside berberine isn't likely to cause big problems.
