Colchicine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP3A4.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.

Here's the gist with berberine and colchicine. Your body relies on a liver enzyme called CYP3A4 to break colchicine down and clear it out. Berberine appears to slow that enzyme down, so colchicine can hang around longer and build up to higher levels than expected.

Why that matters: colchicine has a fairly narrow safety window, meaning the helpful dose and the toxic dose aren't far apart. If levels creep up, you may notice more stomach upset, diarrhea, nausea, or muscle weakness, and at higher levels it can become genuinely dangerous. The good news is this is mostly theoretical and lab-based for colchicine specifically, so it may not happen to everyone. Still, please don't pair these without checking with your pharmacist or doctor first, especially if your kidneys or liver aren't at their best.

About Colchicine

What Is Colchicine Used For?

Brand names: Colchicine, Colsalide Improved, Mitigare, Colcrys, Gloperba, Lodoco

Colchicine tablets and capsules are used to prevent gout flares in adults, and colchicine tablets are also used to treat acute gout flares when taken at the first sign of a flare. Colchicine tablets are additionally approved for familial Mediterranean fever (FMF) — a hereditary inflammatory condition — in adults and children 4 years and older.

Lodoco (colchicine tablet) is specifically indicated to reduce the risk of heart attack, stroke, coronary revascularization, and cardiovascular death in adults with established atherosclerotic disease or multiple cardiovascular risk factors. Colchicine is not a pain reliever and should not be used for pain from other causes.
