Why this applies to Testosterone
Testosterone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for testosterone. Your body partly clears testosterone through a liver enzyme called CYP3A4, and berberine appears to slow that enzyme down a bit. In theory, that could let a little more testosterone hang around in your blood than usual, which might nudge up the chance of side effects like acne, oily skin, mood changes, fluid retention, or a rise in red blood cell counts.
I want to be honest about how solid this is. The concern is mostly theoretical and based on lab and early human data, so it may not amount to much for everyone, and gels or injections aren't broken down exactly the same way as a pill. Still, since you're on a hormone, it's worth a quick chat with your pharmacist or doctor before adding berberine.

Testosterone products are used as replacement therapy in men with low or absent natural testosterone. This is called hypogonadism (the body doesn't make enough testosterone).
Labels list two types: primary hypogonadism (a problem in the testicles) and hypogonadotropic hypogonadism (a problem with the brain's pituitary or hypothalamus signals). Some labels, such as Testopel pellets, also mention helping start puberty in carefully selected males with clearly delayed puberty.
