Bismuth and Apixaban

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Apixaban

Apixaban falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Bismuth interacts with.

Theoretically, use of bismuth subgallate or other bismuth salts might reduce the effects of anticoagulant/antiplatelet drugs.
In humans, bismuth subgallate activates factor XII and accelerates the coagulation cascade.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Eliquis (apixaban): it's a blood thinner that works by blocking a clotting protein called factor Xa, which helps prevent dangerous clots and strokes. The theoretical concern with bismuth (the kind found in some popular stomach remedies) is that it may nudge your clotting system in the opposite direction by activating another clotting factor. In plain terms, bismuth could slightly work against what your apixaban is trying to do.

I want to be honest: this is a theoretical interaction based on how these substances behave, not proven harm in people taking apixaban. Occasional use of a stomach product is unlikely to cause trouble for most folks. Still, since apixaban is protecting you from clots, it's worth a quick chat with your pharmacist or doctor before using bismuth regularly. Don't change your apixaban on your own.

From our pharmacist’s overview on the Apixaban page

“St. John's wort can weaken apixaban (Eliquis), speeding how the body clears this blood thinner and leaving you less protected from the strokes and clots it is prescribed to prevent.”

About Bismuth

What is Bismuth?

  • Upset stomach and indigestion
  • Diarrhea
  • Heartburn and nausea
  • Helicobacter pylori treatment (with other drugs)
  • Traveler's diarrhea prevention

Bismuth is a metallic element used in medicine mostly as bismuth subsalicylate (the active ingredient in some well-known stomach remedies) to ease upset stomach, heartburn, nausea, and diarrhea. It is generally well tolerated for short-term use, but it can cause harmless black stools or a dark tongue, and because most products contain salicylate, it carries cautions similar to aspirin. Always follow the label and check with a pharmacist before regular use.

About Apixaban

What Is Apixaban Used For?

Brand name: Eliquis

Apixaban tablets (sold as Eliquis and under the generic name apixaban) are used to reduce the risk of stroke and systemic embolism (clots traveling to other parts of the body) in adults with nonvalvular atrial fibrillation (an irregular heart rhythm not caused by a heart valve problem). They are also used to prevent deep vein thrombosis (DVT — a blood clot in the leg) after hip or knee replacement surgery, and to treat DVT and pulmonary embolism (PE — a blood clot in the lungs), as well as to lower the risk of those clots coming back.

Eliquis is additionally approved to treat venous thromboembolism (VTE — blood clots in the veins) and reduce the risk of recurrent VTE in pediatric patients from birth and older, following at least 5 days of initial anticoagulant treatment.
