This combination pill contains aspirin, which gives a mild blood-thinning (antiplatelet) effect by making your platelets less sticky. The concern here is theoretical: bismuth salts may nudge your clotting system in the opposite direction by activating one of the clotting factors. In plain terms, bismuth could slightly blunt aspirin's blood-thinning action.

Honestly, this one is based on lab pharmacology, not strong real-world data, so I wouldn't lose sleep over an occasional dose of a stomach remedy. Keep in mind both bismuth subsalicylate and aspirin contain salicylate, so taking them together stacks that up. If you use bismuth regularly or rely on aspirin to protect your heart, check with your pharmacist or doctor first.

Theoretically, bismuth subsalicylate might have an additive effect with other salicylate-containing drugs.
Dietary supplements often contain bismuth in the form of bismuth subsalicylate. In humans, oral bismuth subsalicylate is hydrolyzed in the stomach to form salicylate and bismuth oxychloride.

Here's the simple version: the bismuth in many stomach remedies is actually bismuth subsalicylate, and once it hits your stomach it breaks down into salicylate, which is the same family as the aspirin in your Anacin with Codeine. So you'd essentially be stacking one salicylate on top of another. This is a theoretical concern based on how these drugs work, not something proven to cause big problems for most people, but it's worth knowing.

Doubling up on salicylate can nudge you toward aspirin-type side effects like stomach upset, heartburn, or ringing in the ears, and at higher amounts, easier bruising or bleeding. The codeine and caffeine pieces aren't really the issue here. An occasional dose probably won't hurt, but if you use the stomach product regularly, check with your pharmacist first.

Bismuth is a metallic element used in medicine mostly as bismuth subsalicylate (the active ingredient in some well-known stomach remedies) to ease upset stomach, heartburn, nausea, and diarrhea. It is generally well tolerated for short-term use, but it can cause harmless black stools or a dark tongue, and because most products contain salicylate, it carries cautions similar to aspirin. Always follow the label and check with a pharmacist before regular use.
