Here's the idea behind this one. Defibrotide is given in the hospital to help keep blood flowing and prevent clots in people with liver veno-occlusive disease after a stem cell transplant. Bismuth salts (like the bismuth in some stomach remedies) may nudge the clotting system in the opposite direction. In people, bismuth subgallate has been shown to activate factor XII, which can speed up clotting. In theory, that could work against what defibrotide is trying to do and blunt its clot-preventing benefit.

I'll be honest, this is a theoretical concern based on how these agents behave, not something proven in patients. Also, defibrotide is given in a closely monitored hospital setting, so your transplant team is already watching your clotting and bleeding carefully. Still, mention any bismuth products to your care team rather than adding them on your own.

Bismuth is a metallic element used in medicine mostly as bismuth subsalicylate (the active ingredient in some well-known stomach remedies) to ease upset stomach, heartburn, nausea, and diarrhea. It is generally well tolerated for short-term use, but it can cause harmless black stools or a dark tongue, and because most products contain salicylate, it carries cautions similar to aspirin. Always follow the label and check with a pharmacist before regular use.
