ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Ibuprofen lysine

Ibuprofen lysine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Bismuth interacts with.

Theoretically, use of bismuth subgallate or other bismuth salts might reduce the effects of anticoagulant/antiplatelet drugs.
In humans, bismuth subgallate activates factor XII and accelerates the coagulation cascade.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with bismuth (the metal in many stomach remedies) and ibuprofen lysine. Ibuprofen isn't a blood thinner in the usual sense, but as an NSAID it does mildly affect platelets and your blood's ability to clot. The concern on file is that bismuth might nudge clotting in the opposite direction by activating part of the clotting cascade, theoretically pulling against the mild anti-clotting tilt ibuprofen gives you.

Honestly, this one is very theoretical. It's based on how these things behave in the lab and body chemistry, not on real cases of trouble. For most people taking ibuprofen for pain, fever, or arthritis, occasional bismuth shouldn't cause a problem. Just remember many bismuth products also contain salicylate (aspirin-like), so check with your pharmacist before combining them regularly.

  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Ibuprofen lysine page

“Ibuprofen lysine (NeoProfen) is an injectable form of the NSAID ibuprofen, and this list warrants real attention, starting with its six major entries. The dominant pattern is bleeding: 202 of the 268 ingredients on file can thin the blood or slow clotting, which stacks on an NSAID's own tendency to irritate the stomach and blunt platel…”

About Bismuth

What is Bismuth?

  • Upset stomach and indigestion
  • Diarrhea
  • Heartburn and nausea
  • Helicobacter pylori treatment (with other drugs)
  • Traveler's diarrhea prevention

Bismuth is a metallic element used in medicine mostly as bismuth subsalicylate (the active ingredient in some well-known stomach remedies) to ease upset stomach, heartburn, nausea, and diarrhea. It is generally well tolerated for short-term use, but it can cause harmless black stools or a dark tongue, and because most products contain salicylate, it carries cautions similar to aspirin. Always follow the label and check with a pharmacist before regular use.
