Here's the gist for cilostazol (Pletal): it's an antiplatelet medicine, meaning it helps keep your blood from clotting too easily so blood flows better to your legs. Bismuth (the kind in some stomach remedies) may, at least in theory, nudge your blood toward clotting a bit more by activating part of the clotting cascade. So the concern is that bismuth could slightly work against cilostazol and make it a touch less effective.

I want to be honest, though: this one is theoretical, based on how these substances behave in the lab rather than real reports of problems. An occasional dose for an upset stomach is unlikely to matter much. If you find yourself reaching for bismuth often, just give your pharmacist or doctor a quick heads-up first.

Bismuth is a metallic element used in medicine mostly as bismuth subsalicylate (the active ingredient in some well-known stomach remedies) to ease upset stomach, heartburn, nausea, and diarrhea. It is generally well tolerated for short-term use, but it can cause harmless black stools or a dark tongue, and because most products contain salicylate, it carries cautions similar to aspirin. Always follow the label and check with a pharmacist before regular use.
