Acetaminophen, Aspirin, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Bismuth interacts with.
In humans, bismuth subgallate activates factor XII and accelerates the coagulation cascade.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing to know about this combo: the aspirin in Excedrin is the part that matters. Aspirin works as a mild blood thinner by keeping your platelets from clumping. Bismuth (the kind in stomach remedies) has been shown to nudge your clotting system in the opposite direction, so in theory it could slightly blunt aspirin's blood-thinning effect.
I want to be honest, though: this is a theoretical concern based on how these two work, not something we see causing real problems for most folks. For an occasional headache dose, it's very unlikely to be an issue. If you take aspirin daily specifically to protect your heart, or you use bismuth products regularly, just give your pharmacist a heads-up so we can keep an eye on things.

Dietary supplements often contain bismuth in the form of bismuth subsalicylate. In humans, oral bismuth subsalicylate is hydrolyzed in the stomach to form salicylate and bismuth oxychloride.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the key thing to know: products like Excedrin Migraine already contain aspirin, which is a salicylate. Bismuth subsalicylate (think the pink stomach remedy) breaks down in your stomach into salicylate too. So if you take both around the same time, you're essentially stacking two sources of salicylate, and the totals can add up higher than you intended.
For most healthy folks taking an occasional dose, this is more of a theoretical concern than a real danger. But pushing your salicylate load up can mean more stomach irritation, heartburn, or in larger amounts, ringing in the ears. If you take blood thinners, have ulcers, or use these regularly, please check with your pharmacist or doctor before pairing them.

Bismuth is a metallic element used in medicine mostly as bismuth subsalicylate (the active ingredient in some well-known stomach remedies) to ease upset stomach, heartburn, nausea, and diarrhea. It is generally well tolerated for short-term use, but it can cause harmless black stools or a dark tongue, and because most products contain salicylate, it carries cautions similar to aspirin. Always follow the label and check with a pharmacist before regular use.
