Fedratinib Hydrochloride falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Kava interacts with.

Theoretically, kava might increase levels of CYP2C19 substrates.
In vitro research shows that kava significantly inhibits CYP2C19 enzymes. This effect has not yet been reported in humans.

Here's the short version on combining kava with your fedratinib (Inrebic). Your body uses an enzyme called CYP2C19 to help process fedratinib. Lab studies suggest kava can slow that enzyme down. In theory, that could let more fedratinib build up in your system, which may mean stronger effects and a higher chance of side effects like nausea, diarrhea, vomiting, tiredness, or muscle aches.

I want to be honest about the evidence though: this is based only on test-tube research and has not been seen in real patients yet. Still, fedratinib is a serious cancer medicine, and kava carries its own risk of liver damage. Please loop in your oncologist or pharmacist before using kava so they can weigh in.

Kava has been linked with over 100 cases of hepatotoxicity. Most cases occur with excessive and prolonged use. There is some concern that kava can adversely affect the liver, especially when used in combination with hepatotoxic drugs.

Here's the concern with pairing kava and fedratinib (Inrebic): both can be hard on your liver. Fedratinib already carries a known risk of liver enzyme changes and liver injury, which is why your care team checks your bloodwork regularly while you're on it. Kava has been tied to over 100 reported cases of liver problems, mostly with heavy or long-term use. Stacking the two could, in theory, raise the chance of liver trouble.

To be clear, this is based on anecdotal reports, not strong studies, so it's a caution rather than a definite danger. Still, with a drug this serious for your liver, I'd skip the kava unless your oncologist or pharmacist okays it. Watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and report them right away.

Fedratinib Hydrochloride also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

It is unclear if kava inhibits CYP3AA; research is conflicting.
Although in vitro research suggests that kava inhibits CYP3A4, more robust clinical evidence shows that kava has no effect on CYP3A4. In a clinical study in healthy volunteers, taking kava 1000 mg twice daily (containing a daily dose of 138 mg kavalactones) for 28 days had no effect on CYP3A4 activity.

Here's the short version on combining kava with fedratinib (Inrebic). Fedratinib is cleared from your body partly by a liver enzyme called CYP3A4. The worry on paper is that kava might slow that enzyme down, which could let fedratinib build up and raise the odds of side effects like nausea, diarrhea, fatigue, or low blood counts. But here's the reassuring part: while early test-tube studies hinted kava could block CYP3A4, a better human study found kava had no real effect on this enzyme. So in practice, a meaningful interaction looks unlikely.

I'd still flag one bigger concern. Kava has been tied to rare liver damage, and fedratinib already requires liver monitoring. Please check with your oncology team or pharmacist before adding kava.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
