5-htp and Almotriptan

Here is the interaction we found between these two, with the evidence behind it.

Moderate

SEROTONERGIC DRUGS

Why this applies to Almotriptan

Almotriptan falls under SEROTONERGIC DRUGS — the group 5-htp interacts with.

Combining serotonergic drugs with 5-HTP might cause additive serotonergic effects.
5-HTP can increase serotonin levels and cause serotonergic effects. Theoretically, combining serotonergic drugs with 5-HTP might increase the risk of serotonergic side effects, including serotonin syndrome and cerebral vasoconstrictive disorders. However, serotonin syndrome with 5-HTP has not yet been reported in humans. Monitor patients for signs of serotonin syndrome and other serotonergic side effects if using 5-HTP with serotonergic drugs.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Almotriptan (Axert) works by acting on serotonin receptors to calm down a migraine. Since 5-HTP raises your body's serotonin levels too, taking the two together stacks serotonin activity on top of serotonin activity. In theory that could nudge up the odds of serotonin syndrome, where too much serotonin causes things like agitation, a racing heart, sweating, shivering, muscle twitching, or feeling flushed.

I want to be honest about the evidence here: this concern is theoretical, based on how both substances behave, and serotonin syndrome from 5-HTP hasn't actually been reported in people. So it's worth being aware of, not panicking over. Before combining them, check with your pharmacist or doctor, and call right away if you feel those symptoms after a dose.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

From our pharmacist’s overview on the Almotriptan page

“St. John's wort sits at the top of my worry list for anyone taking almotriptan (Axert, Almogran) for migraines, because it revs up the CYP3A4 enzyme that clears the drug, which can leave you with less relief when the headache hits.”

5-HTP is a compound your body uses to make serotonin, and people take it as a supplement hoping to improve mood, sleep, and headaches.
