St. John's wort sits at the top of my worry list for anyone taking almotriptan (Axert, Almogran) for migraines, because it revs up the CYP3A4 enzyme that clears the drug, which can leave you with less relief when the headache hits. Grapefruit does the reverse, blocking that same enzyme so almotriptan levels climb, and both are among the four major-rated entries on a list of 154 ingredients. This is one to approach with genuine care.
