Adrafinil + Almotriptan
Almotriptan brand names: Axert, Almogran.
Adrafinil has a moderate interaction with Almotriptan. Theoretically, adrafanil might reduce levels and clinical effects of CYP3A4 substrates.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Almotriptan
Almotriptan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Adrafinil interacts with.
Adrafinil is a precursor to the stimulant prescription drug modafinil. Modafinil has been shown to weakly induce CYP3A4 activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two together. Almotriptan, your migraine rescue medicine, is partly broken down by a liver enzyme called CYP3A4. Adrafinil turns into modafinil in your body, and modafinil can mildly speed up that same enzyme. In theory, that could clear almotriptan a little faster and leave you with less migraine relief, meaning your headache, nausea, or light sensitivity might not ease up as well as usual.
I want to be honest though: this is based on how the drugs work on paper, not on real patient reports, and modafinil's effect on this enzyme is weak. So it may not amount to much. Still, since adrafinil is unregulated and rough on the liver, please run it by your pharmacist or doctor before combining them.

By severity: 4 major · 108 moderate · 42 minor.
From our pharmacist’s overview on the Almotriptan page
“St. John's wort sits at the top of my worry list for anyone taking almotriptan (Axert, Almogran) for migraines, because it revs up the CYP3A4 enzyme that clears the drug, which can leave you with less relief when the headache hits.”

About Almotriptan
What Is Almotriptan Used For?
Brand names: Axert, Almogran
Almotriptan tablets are used to treat migraine headache attacks — with or without aura — as they happen. They are not for preventing migraines before they start.
In adults, almotriptan tablets treat acute migraine attacks. In adolescents ages 12 to 17, almotriptan tablets treat acute migraine headache pain in those who typically have attacks lasting 4 hours or more when untreated. Note that in adolescents, the effect on migraine-related symptoms such as nausea and light sensitivity was not established in clinical trials. Almotriptan is not indicated for cluster headache or for hemiplegic or basilar migraine.
