Why this applies to Almotriptan

Almotriptan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Hops interacts with.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the situation with hops and your migraine medicine. Almotriptan is partly cleared from your body by a liver enzyme called CYP3A4. Hops might nudge that enzyme to work a little faster, which in theory could clear almotriptan slightly quicker and make it a touch less effective for stopping a migraine. Almotriptan is the active drug itself (not a prodrug), so faster breakdown would mean weaker, not stronger, effects.

The honest part: this is built on lab and animal data, and a human study showed only about a 7.6% change in a similar drug, which is too small to really notice. I wouldn't lose sleep over it. Still, if your migraines aren't responding like usual, mention it to your pharmacist or doctor.

Hops are most often used for sleep and mild anxiety, frequently combined with valerian, but the human evidence is limited and not very strong. They are generally well tolerated when used short-term, though they may cause drowsiness, and should be avoided during pregnancy and breastfeeding because their safety is unknown.

About Almotriptan

What Is Almotriptan Used For?

Brand names: Axert, Almogran

Almotriptan tablets are used to treat migraine headache attacks — with or without aura — as they happen. They are not for preventing migraines before they start.

In adults, almotriptan tablets treat acute migraine attacks. In adolescents ages 12 to 17, almotriptan tablets treat acute migraine headache pain in those who typically have attacks lasting 4 hours or more when untreated. Note that in adolescents, the effect on migraine-related symptoms such as nausea and light sensitivity was not established in clinical trials. Almotriptan is not indicated for cluster headache or for hemiplegic or basilar migraine.
