Here's the simple version: Xanax (alprazolam) already slows down activity in your brain to ease anxiety and panic. Hops, when taken as a supplement for sleep or relaxation, may have its own mild calming effect. Put the two together and those effects could stack up, leaving you more drowsy, foggy, dizzy, or unsteady on your feet than you'd expect from the Xanax alone.

I'll be honest about the evidence though: this concern comes mostly from animal studies and how these compounds work in theory, not from strong human research. So it's a maybe, not a sure thing. Still, since both can make you sleepy, it's worth a quick chat with your pharmacist or doctor before combining them, especially before driving.

Alprazolam also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, hops extract might alter metabolism of CYP3A4 substrates; however, this effect may not be clinically significant.
Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.

Here's the gist for alprazolam (Xanax) and hops. Your liver uses an enzyme called CYP3A4 to clear alprazolam out of your body. Some lab and animal studies hint that hops might speed that enzyme up a little, which in theory could lower your alprazolam levels and make it work slightly less well. Alprazolam is the active drug itself, not a prodrug, so faster clearance means a touch less effect, not more.

The good news: a real human study found hops dropped alprazolam levels by only about 7.6%, which is tiny and unlikely to change how you feel or how well it controls your anxiety or panic. One thing worth watching is that both can cause drowsiness, so you may feel extra sleepy together. Check with your pharmacist before combining them.

What Is Alprazolam Used For?

Brand names: Xanax, Niravam

Alprazolam treats generalized anxiety disorder (GAD) and panic disorder, with or without agoraphobia, in adults. These uses are supported across multiple alprazolam products, including tablets, orally disintegrating tablets, oral solution, and extended-release tablets.

Xanax (tablet) is indicated for the acute treatment of GAD and for panic disorder. Xanax XR (extended-release tablet) and alprazolam extended-release tablets are indicated specifically for panic disorder with or without agoraphobia — not for GAD. Alprazolam orally disintegrating tablets and alprazolam oral solution are indicated for both GAD and panic disorder.
