Here's the thing to know if you take Xanax (alprazolam) and are thinking about St. John's wort. Your body clears alprazolam using a liver enzyme called CYP3A4, and St. John's wort revs that enzyme up. So instead of building up, your alprazolam actually gets broken down faster and your blood levels drop.

In plain terms, your Xanax may stop working as well. You could feel your anxiety or panic symptoms creeping back, even on your usual dose. This isn't a guess from a test tube only, it's been seen in people, which is why it's rated as a serious interaction. Please don't change anything on your own. Talk with your pharmacist or doctor first so they can guide you safely.

St. John's wort increases the clearance of alprazolam and decreases its effects.
Alprazolam, which is used as a probe for cytochrome P450 3A4 (CYP3A4) activity, has a two-fold increase in clearance when given with St. John's wort. St. John's wort reduces the half-life of alprazolam from 12.4 hours to 6 hours.

Here's the practical takeaway for combining St. John's wort with Xanax (alprazolam). This herb revs up a liver enzyme called CYP3A4, which is the same pathway your body uses to clear alprazolam. In a clinical study, taking the two together roughly doubled how fast alprazolam was removed, cutting its half-life from about 12 hours down to 6. In plain terms, the herb pushes the drug out of your system faster.

The likely result is that your Xanax just doesn't work as well. You might notice your anxiety or panic symptoms creeping back, or shorter relief between doses. Alprazolam isn't a prodrug, so there's no flip here, weaker effect is the real concern. Please don't change anything on your own. Loop in your pharmacist or doctor before mixing these two.

What Is Alprazolam Used For?

Brand names: Xanax, Niravam

Alprazolam treats generalized anxiety disorder (GAD) and panic disorder, with or without agoraphobia, in adults. These uses are supported across multiple alprazolam products, including tablets, orally disintegrating tablets, oral solution, and extended-release tablets.

Xanax (tablet) is indicated for the acute treatment of GAD and for panic disorder. Xanax XR (extended-release tablet) and alprazolam extended-release tablets are indicated specifically for panic disorder with or without agoraphobia — not for GAD. Alprazolam orally disintegrating tablets and alprazolam oral solution are indicated for both GAD and panic disorder.
