Acetaminophen, Caffeine, Isometheptene falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Hops interacts with.
In vitro research suggests that flavonoid constituents of hops inhibit CYP1A2 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, does not affect levels of caffeine, a CYP1A2 probe substrate.

Good question about pairing hops with Migralam (which combines acetaminophen, caffeine, and isometheptene). The caffeine in this combo is processed by a liver enzyme called CYP1A2. In a lab dish, some hops compounds appear to slow that enzyme down, which in theory could let caffeine linger a bit longer and add to jittery side effects like a racing heart, restlessness, or trouble sleeping.
Here's the reassuring part: when researchers actually tested a standardized hops extract in real people, it did not change caffeine levels at all. So this is really just a theoretical, lab-only concern, rated minor and unlikely. CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Animal research suggests that specific constituents of hops, called lupulones, can induce hepatic CYP3A4 enzyme activity. However, a pharmacokinetic study in healthy postmenopausal patients with normal metabolism shows that taking a standardized extract of spent hops containing prenylated phenols, as 59.5 mg twice daily for 2 weeks, decreases the concentration of alprazolam, a CYP3A4 probe substrate, by 7.6%. This reduction is unlikely to be clinically relevant.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version on combining hops with Migralam (a mix of acetaminophen, caffeine, and isometheptene): the concern is mostly theoretical. Lab and animal studies hint that hops compounds may speed up a liver enzyme called CYP3A4, which helps process some of these ingredients. If that happened in people, it could in theory clear the drug a little faster and slightly soften its effect, but this isn't a prodrug situation, so you wouldn't expect a dangerous buildup.
In reality, a human study found only about an 8% change with a related medication, which is too small to matter. I wouldn't lose sleep over this pairing. Still, since hops can also make you drowsy, it's worth a quick chat with your pharmacist or doctor if you use it regularly.
