Almotriptan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for migraine sufferers: your body clears almotriptan partly through a liver enzyme called CYP3A4. Some lab studies suggest certain batches of melatonin can slow that enzyme down a bit. If that happened, almotriptan might hang around in your system a little longer, which could nudge up its usual effects, things like sleepiness, dizziness, dry mouth, or a tight, flushing feeling.

That said, I want to keep this in perspective. This is rated a minor interaction based only on test-tube research, not real patient data, so it's more theoretical than proven. Most people use both occasionally without trouble. Still, if you take melatonin nightly and use almotriptan often, it's worth a quick chat with your pharmacist or doctor to be safe.

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

About Almotriptan

What Is Almotriptan Used For?

Brand names: Axert, Almogran

Almotriptan tablets are used to treat migraine headache attacks — with or without aura — as they happen. They are not for preventing migraines before they start.

In adults, almotriptan tablets treat acute migraine attacks. In adolescents ages 12 to 17, almotriptan tablets treat acute migraine headache pain in those who typically have attacks lasting 4 hours or more when untreated. Note that in adolescents, the effect on migraine-related symptoms such as nausea and light sensitivity was not established in clinical trials. Almotriptan is not indicated for cluster headache or for hemiplegic or basilar migraine.
