Why this applies to Miconazole

Miconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the practical picture: the worry on file is that melatonin might slow down CYP3A4, a liver enzyme that breaks down many medicines. In theory, that could let a CYP3A4 drug build up a bit. With miconazole, though, this barely applies in real life. Most people use it as a cream, suppository, or powder (think Monistat or Zeasorb-AF) for yeast or skin fungus, so very little gets into your bloodstream.

On top of that, this concern comes only from lab studies, not from people, and it's rated minor. So for topical or vaginal miconazole, I wouldn't lose sleep over it. If you ever take an oral form, just mention both to your pharmacist so they can take a quick look.

The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
