When you take hydrocortisone for something like arthritis flares, severe allergies, or an autoimmune condition such as lupus, part of the goal is to calm down an overactive immune system. The concern here is a bit of a tug-of-war: melatonin can gently nudge the immune system in the opposite direction, toward being more active. In theory, that could work against what your hydrocortisone is trying to do.

Here's the honest part though. This interaction is mostly theoretical and based on weaker evidence, so for most people taking short-term melatonin for sleep, it isn't likely to cause an obvious problem. Still, if you rely on hydrocortisone to keep a serious condition under control, it's worth a quick chat with your pharmacist or doctor before adding melatonin, just so everyone's on the same page.

Hydrocortisone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for melatonin and hydrocortisone: your liver uses an enzyme called CYP3A4 to help break hydrocortisone down. Some lab studies hint that melatonin might slow that enzyme a little. If that happened, your body could clear hydrocortisone more slowly, leaving slightly higher levels around. In theory, that might nudge up steroid-type effects like fluid retention, a bump in blood sugar, mood or sleep changes, or stomach upset.

That said, I want to keep this in perspective. This is a theoretical concern based on test-tube research, not real patient reports, and it's rated minor. For most people taking occasional melatonin, this is unlikely to cause trouble. Still, if you take hydrocortisone daily for adrenal replacement or a chronic condition, it's worth a quick chat with your pharmacist or doctor before adding melatonin.

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

What Is Hydrocortisone Used For?

Brand names: Cortef, Hydrocortone, Carmol HC cream, Hydrocortisone Iodoquinol Cream, Vytone Cream, Massengil Medicated Towelettes, Dioderm, Efcortelan Cream, Mildison, Alphaderm, Calmurid HC, Efcortelan Ointment, Texacort, Alkindi Sprinkle

Hydrocortisone is used to calm skin inflammation and relieve itching caused by conditions that respond to corticosteroid treatment. In adults, it treats a broad range of inflammatory skin conditions. In children as young as 3 months old, it is specifically approved for mild to moderate atopic dermatitis (eczema).

It is also used in rectal foam form to treat inflammation of the lower bowel, such as ulcerative proctitis.
