Galantamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP2D6.
Laboratory research suggests that certain lots of melatonin inhibit CYP2D6. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the simple version: your body partly clears galantamine using a liver enzyme called CYP2D6. Some lab studies hint that certain batches of melatonin might slow that enzyme down a bit. If that happened in real life, galantamine could build up slightly, and you might notice more of its usual side effects, things like nausea, an upset stomach, loss of appetite, dizziness, or a slower heartbeat.

I want to be honest about how shaky this is, though. This warning comes only from test-tube research, not from people, and it's rated as a minor, possible interaction. For most folks, short-term melatonin for sleep is unlikely to cause a problem here. Still, since galantamine is a serious medicine, run it by your pharmacist or doctor before pairing the two.

Here's the gist with these two together. Galantamine is partly cleared by a liver enzyme called CYP3A4. Some lab samples of melatonin have slowed that enzyme down a bit. In theory, if melatonin nudges CYP3A4 to work slower, a little more galantamine could linger in your system. With galantamine that might mean more of its usual side effects, like nausea, vomiting, loss of appetite, dizziness, or a slower heart rate.

I want to keep this in perspective, though. This is only a theoretical concern from test-tube research, it's rated minor, and it hasn't been clearly seen in real patients. Most folks tolerate the combo fine. Still, since galantamine can affect the heart, it's worth a quick chat with your pharmacist or doctor before adding melatonin.

Galantamine — available as extended-release capsules, tablets, and an oral solution — is used to treat mild to moderate dementia caused by Alzheimer's disease. All supported formulations share this indication.

It does not cure or slow the underlying disease, but it can help support cognitive function in people with early to moderate-stage Alzheimer's. Its benefits may lessen as the disease progresses and more brain cells are lost.
