Acetaminophen, Caffeine, Isometheptene falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP1A2. Also, other CYP1A2 substrates might decrease the metabolism of melatonin, increasing melatonin levels.
Melatonin is metabolized in the liver primarily by the CYP2C19 and CYP1A2 enzymes. Theoretically, combined administration of melatonin with drugs metabolized by the CYP1A2 enzyme might reduce the metabolism of these drugs, resulting in increased serum levels. Conversely, some drugs metabolized by CYP1A2 may inhibit the metabolism of melatonin, resulting in increased serum levels of melatonin. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

The piece of Migralam that interacts here is the caffeine. Your liver uses an enzyme called CYP1A2 to clear caffeine, and melatonin can theoretically slow that same enzyme down a little. If caffeine is cleared more slowly, it can hang around longer, so you might notice a bit more jitteriness, a racing or pounding heart, or trouble actually falling asleep, which is a little ironic when you're taking melatonin to sleep.

Honestly, this one is on the gentle side. The concern is theoretical and based on how these drugs work, not on strong real-world reports. None of this is me saying to stop anything. If you take Migralam for migraines and also use melatonin, just mention it to your pharmacist or doctor so they can weigh in.

Theoretically, taking caffeine with melatonin might increase levels of melatonin.
Some evidence suggests that caffeine consumption can decrease endogenous melatonin levels, while other evidence suggests that caffeine increases endogenous melatonin levels. When administered in combination with melatonin supplements, caffeine seems to increase melatonin effects and levels. The reason for this discrepancy is not completely clear. Part of the discrepancy may result from the fact that caffeine can inhibit melatonin synthesis as well as inhibit melatonin metabolism. By functioning as an adenosine receptor antagonist, caffeine may indirectly inhibit the synthesis of melatonin. Conversely, because melatonin and caffeine are both metabolized by cytochrome P450 1A2 (CYP1A2) enzyme, concomitant use of melatonin and caffeine may reduce the metabolism of melatonin, resulting in higher serum levels.

Here's the wrinkle with Migralam: it contains caffeine, and caffeine and melatonin both get processed by the same liver enzyme (CYP1A2). When you take them together, the caffeine can slow down how fast melatonin is cleared, so your melatonin levels may run a little higher than expected. In plain terms, the melatonin's sleepy, wind-down effect might feel a bit stronger.

This is mostly a theoretical, modest interaction, and most people tolerate it fine. The bigger practical point is that Migralam has caffeine in it, which can fight against melatonin's purpose by keeping you alert. Taking a caffeine-containing migraine product near bedtime may make it harder to fall asleep regardless. If you're using both, just mention it to your pharmacist or doctor so the timing makes sense for you.

Here's the gist with Migralam (acetaminophen, caffeine, and isometheptene) and melatonin. In some lab studies, certain batches of melatonin slowed down a liver enzyme called CYP3A4. Since parts of this combo can be handled by that enzyme, the theory is that melatonin might let a little more of the drug build up in your blood. If that happened, you could in theory notice the caffeine side a bit more, things like jitteriness, a racing heart, or trouble settling down to sleep.

I want to be honest, though: this is based only on test-tube and animal research, not on real patients, and it's rated as a minor interaction. For most people taking Migralam occasionally for a headache, it's unlikely to cause real trouble. Still, run it by your pharmacist or doctor if you use both regularly.
