Fosamprenavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist for these two together. Fosamprenavir is actually a prodrug: your body converts it into amprenavir, the active form that fights HIV, and that active drug is handled by an enzyme called CYP3A4. The theory on file is that some batches of melatonin may slow down CYP3A4. If that happened, levels of the active drug could nudge upward, which in theory might mean a bit more chance of side effects like nausea, loose stools, headache, or skin rash.

I want to be honest about the evidence, though. This is based only on lab testing, not on real people, and it's rated as a minor interaction. For most folks taking occasional melatonin for sleep, it's unlikely to cause trouble. Since HIV medications are too important to guess with, just give your pharmacist or doctor a heads-up before you start melatonin.

What Is Fosamprenavir Used For?

Brand names: Lexiva, Telzir

Fosamprenavir tablets (sold as Fosamprenavir Calcium and Lexiva) are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children as young as 4 weeks old. It is not used alone — it is always part of a combination regimen.

For adults who have already taken protease inhibitor medicines before, fosamprenavir is used alongside a low dose of ritonavir. Once-daily dosing with ritonavir is not recommended for adults who have previously used protease inhibitors, or for any pediatric patients.
