Here's the gist for this pair. Once your liver activates adrafinil into modafinil, that modafinil is cleared in part by an enzyme called CYP3A4. Fosamprenavir (Lexiva) acts as a CYP3A4 blocker, so in theory it could slow the breakdown of modafinil and let it build up. That might mean stronger stimulant effects: trouble sleeping, a racing or pounding heart, jitteriness, anxiety, headache, or higher blood pressure.

I want to be honest that this concern is theoretical, based on how these drugs behave on paper rather than real patient reports. Even so, adrafinil is unregulated and hard on the liver, and HIV therapy is too important to risk muddying up. Please loop in your doctor or pharmacist before combining them.

Here's the concern with pairing these two. Adrafinil turns into modafinil in your body, and modafinil can gently speed up a liver enzyme called CYP3A4. Your HIV medication, fosamprenavir, leans on that same enzyme. If it gets cleared a little faster than usual, the active drug levels in your blood could dip.

Why that matters: with HIV treatment, you want steady, reliable drug levels to keep the virus suppressed. Lower levels could mean less control over the virus and, over time, a risk of resistance. I want to be straight with you, though. This interaction is theoretical, based on how the drugs work rather than real-world cases, and modafinil is only a weak booster of that enzyme. Still, don't combine them on your own. Please check with your HIV provider or pharmacist first.

About Fosamprenavir

What Is Fosamprenavir Used For?

Brand names: Lexiva, Telzir

Fosamprenavir tablets (sold as Fosamprenavir Calcium and Lexiva) are used together with other antiretroviral medicines to treat HIV-1 infection in adults and in children as young as 4 weeks old. It is not used alone — it is always part of a combination regimen.

For adults who have already taken protease inhibitor medicines before, fosamprenavir is used alongside a low dose of ritonavir. Once-daily dosing with ritonavir is not recommended for adults who have previously used protease inhibitors, or for any pediatric patients.
