Melatonin + Dronabinol
Dronabinol brand names: Marinol, Syndros.
Melatonin has a minor interaction with Dronabinol. Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Dronabinol
Dronabinol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for dronabinol (Marinol, Syndros): your liver clears it largely through an enzyme called CYP3A4. Some lab samples of melatonin appear to slow that same enzyme down a bit. If that happened in your body, dronabinol could clear a little more slowly and build up slightly, which in theory might make its effects, and side effects, a touch stronger. With dronabinol that can mean more drowsiness, dizziness, a feeling of being high or confused, or a fast heartbeat.
I want to be honest about the strength of this, though. It comes only from test-tube research, not from real patients, and it's rated as minor. For most people this won't cause a noticeable problem. Still, since both can make you sleepy, it's worth a quick chat with your pharmacist or doctor before combining them.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Dronabinol page
“A glass of grapefruit juice can push dronabinol (Marinol, Syndros), the prescription form of THC used for chemotherapy nausea and appetite loss, to higher blood levels than intended, amplifying dizziness, sedation, and its mind-altering effects. That is one of three major-rated interactions among the 151 supplements and herbs on file,…”

About Melatonin
What is Melatonin?
- Trouble falling asleep (insomnia)
- Jet lag
- Delayed sleep phase syndrome
- Non-24-hour sleep-wake disorder
- Shift-work sleep problems
- Sleep problems in some children with conditions like autism or ADHD
Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
About Dronabinol
What Is Dronabinol Used For?
Brand names: Marinol, Syndros
Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.
Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
