ANTIHYPERTENSIVE DRUGS
Why this applies to Hydrochlorothiazide
Hydrochlorothiazide falls under ANTIHYPERTENSIVE DRUGS — the group Melatonin interacts with.
Some clinical research suggests that taking melatonin decreases blood pressure in healthy adults. Also, melatonin seems to lower systolic and diastolic blood pressure in individuals with high blood pressure at nighttime or untreated essential hypertension. However, melatonin seems to worsen blood pressure in patients who are taking antihypertensive medications. Immediate-release melatonin 5 mg at night in combination with nifedipine GITS (Procardia XL) increases systolic blood pressure an average of 6.5 mmHg, diastolic blood pressure by an average of 4.9 mmHg, and heart rate by 3.9 bpm. Also, results from animal research suggest that melatonin reduces the effectiveness of certain antihypertensive drugs, including methoxamine and clonidine.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your blood pressure pill: melatonin and hydrochlorothiazide can tug on blood pressure in opposite ways, and the result isn't fully predictable. On its own, melatonin tends to nudge blood pressure down a bit. But in people already taking blood pressure medicine, some research found melatonin actually blunted the medicine's effect, meaning your numbers could run a little higher than expected.
For hydrochlorothiazide specifically, this isn't about your liver breaking down the drug. It's about the overall blood pressure response. The studied effect was modest, and much of the concern is theoretical or from other antihypertensives. Still, if you check your pressure at home, watch for changes after starting melatonin, and run it by your pharmacist or doctor first.

The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

What Is Hydrochlorothiazide Used For?
Brand names: Esidrix, Hydro, Microzide, HydroDiuril, Hydro-D, Oretic, Aquazide H, Aquazide, HCTZ
Hydrochlorothiazide treats high blood pressure (hypertension) in adults and children, either alone or combined with other blood pressure medicines. Lowering blood pressure reduces the risk of strokes and heart attacks.
It also treats edema (fluid buildup) linked to congestive heart failure, hepatic cirrhosis (liver scarring), and kidney conditions including nephrotic syndrome. Hydrochlorothiazide tablets are additionally indicated for edema caused by corticosteroid or estrogen therapy, and edema from acute glomerulonephritis or chronic renal failure. Inzirqo (the oral suspension) is specifically approved for both hypertension and edema in adult and pediatric patients.
