Melatonin + Itraconazole
Itraconazole brand names: Sporanox, Sporanox Injection, Tolsura.
Melatonin has a minor interaction with Itraconazole. Theoretically, melatonin might increase levels of drugs metabolized by CYP3A4.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Itraconazole
Itraconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for itraconazole (Sporanox, Tolsura). The concern on file is that melatonin might slightly slow down CYP3A4, one of the liver enzymes that helps clear itraconazole. If that enzyme works a bit slower, itraconazole could in theory build up a little, which might mean more of its usual side effects, things like nausea, stomach upset, headache, or in rare cases effects on the liver or heart rhythm.
The honest part: this is a theoretical interaction based only on lab studies, and it's rated minor. Real-world problems are unlikely for most people. Still, itraconazole already interacts with a long list of medicines, so it's smart to mention any melatonin use to your pharmacist or doctor before combining them.

What These Ratings Mean
Every interaction above is graded three ways — here’s the key to each label.
Clinically significant. Avoid the combination, or use only under close professional supervision.
Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.
Generally not serious. Be aware of it and mention it to your pharmacist.
- LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
- ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
- PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
- UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Itraconazole page
“Serious fungal infections are what itraconazole (Sporanox, Tolsura) is for, and the drug travels through CYP3A4, the liver enzyme that also handles a large share of supplements. That overlap is why this list reaches 182 ingredients, and it earns genuine care; start with the six rated major.”

About Melatonin
What is Melatonin?
- Trouble falling asleep (insomnia)
- Jet lag
- Delayed sleep phase syndrome
- Non-24-hour sleep-wake disorder
- Shift-work sleep problems
- Sleep problems in some children with conditions like autism or ADHD
Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.
About Itraconazole
What Is Itraconazole Used For?
Brand names: Sporanox, Sporanox Injection, Tolsura
Itraconazole capsules (including Sporanox capsules and generic itraconazole capsules) treat serious fungal infections — blastomycosis, histoplasmosis, and aspergillosis — in both people with normal immune systems and those who are immunocompromised. Capsule forms are also approved for nail fungus (onychomycosis) caused by dermatophytes in non-immunocompromised patients. Tolsura (a gelatin-coated capsule formulation) treats blastomycosis, histoplasmosis, and aspergillosis in adults, but is not approved for nail fungus and is not interchangeable with other itraconazole products.
Itraconazole oral solution is specifically approved for oropharyngeal candidiasis (thrush — a fungal infection of the mouth) and esophageal candidiasis (a fungal infection of the food pipe). Only the oral solution has been shown to work for these mouth and throat infections; capsules cannot be substituted for this use.
