CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Amiodarone
Amiodarone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Melatonin interacts with.
Laboratory research shows that certain lots of melatonin inhibit CYP3A4. Theoretically, combined administration of melatonin with certain drugs metabolized by CYP3A4 may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for these two. Your liver uses an enzyme called CYP3A4 to help break down amiodarone, and some lab studies hint that melatonin may slow that same enzyme down a bit. In theory, that could let amiodarone build up slightly and nudge its effects and side effects upward, things like feeling unusually tired, dizzy, or noticing a slower heartbeat.
I want to be honest about how soft this evidence is. It comes only from test-tube research, not real patients, and the interaction is rated minor. That said, amiodarone is a powerful heart-rhythm drug with a narrow safety window, so even small shifts deserve respect. Please loop in your pharmacist or cardiologist before adding melatonin, rather than starting or stopping anything on your own.

Amiodarone is prescribed for adults with serious, life-threatening heart-rhythm problems that haven't responded to other medications. Specifically, it's used for two dangerous arrhythmias (abnormal heartbeats): recurrent ventricular fibrillation (a chaotic, potentially fatal heart rhythm) and recurrent ventricular tachycardia (a dangerously fast heart rhythm that causes unstable blood pressure).
Because amiodarone comes with significant risks, doctors reserve it for situations where other options haven't worked or can't be tolerated. It is not a first-choice medication — it's a last-resort tool for serious, life-threatening conditions.
