Melatonin and Triamcinolone

Here is the interaction we found between these two, with the evidence behind it.

Moderate

IMMUNOSUPPRESSANTS

Why this applies to Triamcinolone

Triamcinolone falls under IMMUNOSUPPRESSANTS — the group Melatonin interacts with.

Theoretically, melatonin might interfere with immunosuppressive therapy.
Melatonin can stimulate immune function. Theoretically, melatonin might interfere with immunosuppressive therapy.
Severity Moderate Likelihood Possible Evidence B

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the idea behind this pairing. One of the jobs of triamcinolone (a corticosteroid) is to calm down or dial back an overactive immune system, which is how it eases inflammation in things like arthritis, asthma flares, and severe allergies. Melatonin, on the other hand, may gently nudge the immune system in the opposite direction (it can act as a mild immune stimulant). So in theory, melatonin could work against what your steroid is trying to do.

I want to be honest about the strength of this: it's mostly theoretical and based on limited research, not strong human studies. For most people taking triamcinolone short term, occasional melatonin for sleep is unlikely to cause obvious problems. But if you're on this steroid to control a serious immune or inflammatory condition, please check with your pharmacist or doctor first before adding melatonin regularly.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

About Melatonin

What is Melatonin?

Melatonin
  • Trouble falling asleep (insomnia)
  • Jet lag
  • Delayed sleep phase syndrome
  • Non-24-hour sleep-wake disorder
  • Shift-work sleep problems
  • Sleep problems in some children with conditions like autism or ADHD

Melatonin is a hormone your body makes naturally to help control your sleep-wake cycle, and the supplement form is widely used to help with sleep timing problems and jet lag. The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

About Triamcinolone

What Is Triamcinolone Used For?

Brand names: Aristocort, Kenacort, Adcortyl Intra-articular / Intradermal, Kenalog Intra-articular / Intramuscular, Adcortyl in Orabase, Triesence

Triamcinolone is available in many forms — injections, creams, ointments, nasal sprays, dental pastes, and eye injections — each approved for specific uses. Injectable forms given into muscle are used when oral steroids aren't an option, covering severe allergies (like asthma flares, severe skin reactions, drug reactions, and serum sickness), serious skin diseases, certain endocrine (hormonal) disorders, and more.

Injections given directly into a joint (intra-articular) or soft tissue help with conditions like rheumatoid arthritis, gout flares, bursitis, and tendon inflammation. Injections into skin lesions (intralesional) treat conditions such as keloids, alopecia areata, and psoriatic plaques. Triesence (an ophthalmic injection) is specifically indicated for eye conditions like uveitis and temporal arteritis, and for visualization during eye surgery. Topical creams, ointments, lotions, and sprays relieve itching and inflammation from corticosteroid-responsive skin conditions. Dental paste forms (Oralone, Kourzeq) treat painful mouth sores from injury. Nasal sprays temporarily relieve hay fever and upper respiratory allergy symptoms such as runny nose, sneezing, and congestion.
