Hydrochlorothiazide, Lisinopril falls under ANTIHYPERTENSIVE DRUGS — the group Melatonin interacts with.

Theoretically, taking melatonin with antihypertensive drugs might increase the risk of hypotension or hypertension.
Some clinical research suggests that taking melatonin decreases blood pressure in healthy adults. Also, melatonin seems to lower systolic and diastolic blood pressure in individuals with high blood pressure at nighttime or untreated essential hypertension. However, melatonin seems to worsen blood pressure in patients who are taking antihypertensive medications. Immediate-release melatonin 5 mg at night in combination with nifedipine GITS (Procardia XL) increases systolic blood pressure an average of 6.5 mmHg, diastolic blood pressure by an average of 4.9 mmHg, and heart rate by 3.9 bpm. Also, results from animal research suggest that melatonin reduces the effectiveness of certain antihypertensive drugs, including methoxamine and clonidine.

Here's the wrinkle with melatonin and your Zestoretic (lisinopril plus hydrochlorothiazide). On its own, melatonin can gently lower blood pressure. But some research points the other way when it's added to blood pressure medicine. In one study, melatonin combined with a different blood pressure drug actually nudged readings up a bit, by roughly 5 to 6 points, along with a slightly faster heart rate. So the concern here is that melatonin could blunt how well your pill controls your numbers.

This is more theoretical than certain, and many people use both without trouble. Still, if you take them together, it's smart to check your blood pressure at home and let your pharmacist or doctor know before starting melatonin. Don't change either one on your own.

The evidence is strongest for certain circadian (body-clock) disorders and is more mixed for everyday insomnia. It is generally well tolerated for short-term use, but talk with your pharmacist or doctor before starting, especially for children or during pregnancy.

Lisinopril and hydrochlorothiazide tablets — sold as the brand Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events such as stroke and heart attack.

This is a fixed-dose combination product and is not intended as a first-line (initial) treatment for high blood pressure. It is typically prescribed for patients whose blood pressure is not adequately controlled on a single medication alone.
