Galantamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Grape interacts with.
In vitro evidence suggests that grape seed extract might inhibit CYP2D6 enzymes. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with grape seed extract and galantamine. Your body uses an enzyme called CYP2D6 to help clear galantamine. Lab studies hint that concentrated grape seed extract might slow that enzyme down. If that happened, galantamine could build up a bit more than usual, which can mean stronger side effects like nausea, vomiting, diarrhea, dizziness, a slow heartbeat, or loss of appetite.
I want to be honest about the strength of this, though. It's only been seen in test-tube studies, never confirmed in real people, so it's more of a theoretical caution than a proven problem. Eating grapes as food is not a concern. If you're thinking about a grape seed supplement, just run it by your pharmacist or doctor first.

In vitro evidence suggests that grape seed extract might inhibit CYP3A4 enzymes. However, evidence from animal research shows that grape seed extract may induce CYP3A4 in the liver. So far, these interactions have not been reported in humans.

Here's the situation with grape seed extract and your galantamine. Galantamine is partly cleared by a liver enzyme called CYP3A4, and grape seed extract may bump into that same enzyme. The catch is that the science can't agree on which direction it goes. Lab (test tube) studies hint it might slow the enzyme down, which could let galantamine build up and bring more nausea, vomiting, diarrhea, dizziness, or a slow heartbeat. But animal studies suggest the opposite, speeding the enzyme up and possibly weakening the drug's effect.
Honestly, this hasn't been seen in real people, so it's a weak, theoretical concern. Eating grapes won't be an issue, and concentrated supplements are unlikely to cause trouble. Still, check with your pharmacist or doctor before adding a grape seed supplement.
