Theoretically, taking melatonin with methamphetamine may increase the adverse effects of methamphetamine.
Animal research suggests that melatonin exacerbates the adverse effects of methamphetamine, resulting in greater depression of tryptophan hydroxylase (TPH) and tyrosine hydroxylase (TH) activity, as well as a significant reduction in dopamine levels. This has not been shown in humans.

Here's the honest picture with prescription methamphetamine (Desoxyn) and melatonin: the worry comes almost entirely from animal studies, not from people. In those lab studies, combining the two seemed to deepen some of methamphetamine's harmful effects on brain chemicals like dopamine. That signal has not been shown in humans, so I don't want to overstate it.

What this means for you day to day is pretty mild. Melatonin nudges your body toward sleep, while methamphetamine is a stimulant, so on a practical level melatonin may not work as well at bedtime if your dose is still active. I wouldn't expect dangerous interactions at normal sleep doses, but since this is a controlled stimulant, it's worth a quick chat with your pharmacist or prescriber before adding melatonin.

Why this applies to Methamphetamine

Methamphetamine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Melatonin interacts with.

Theoretically, melatonin might increase levels of drugs metabolized by CYP2D6.
Laboratory research suggests that certain lots of melatonin inhibit CYP2D6. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the idea behind this one: prescription methamphetamine (Desoxyn) is partly cleared by a liver enzyme called CYP2D6, and some lab batches of melatonin appear to slow that enzyme down. In theory, a slower enzyme means your body clears the methamphetamine a little less efficiently, so levels could creep up. With a stimulant, that might show up as a faster heartbeat, higher blood pressure, jitteriness, trouble sleeping, or feeling on edge.

I want to be honest about the strength here: this is only seen in test-tube research, not in real people, and it's rated as a minor, possible interaction. For most folks it likely won't cause a noticeable problem. Still, since you're combining a stimulant with a sleep aid, it's worth a quick chat with your pharmacist or doctor before pairing them.
