Brexpiprazole falls under SEIZURE THRESHOLD LOWERING DRUGS — the group Melatonin interacts with.
Some clinical evidence suggests that melatonin may increase the frequency of seizures in certain patients, particularly children with neurological disabilities.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with brexpiprazole (Rexulti) and melatonin. Like many antipsychotics, brexpiprazole can slightly lower the seizure threshold, meaning it can make seizures a touch more likely in people who are already prone to them. The concern on file is that melatonin might add to that effect. This isn't about melatonin changing how your body processes the drug. It's simply a theoretical chance that the two together could nudge seizure risk up a bit.
I want to be honest that this is theoretical, based mostly on how these things work, and the signal is strongest in children with neurological conditions, not most adults. For the average person sleeping poorly, it's usually not a big deal. Still, if you have a seizure history, please run it by your pharmacist or doctor before combining them.
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Laboratory research suggests that certain lots of melatonin inhibit CYP2D6. Theoretically, combined administration of melatonin with certain drugs metabolized by the CYP2D6 enzyme may reduce the metabolism of these drugs, resulting in increased serum levels. Until more is known, use melatonin cautiously in patients taking drugs metabolized by these enzymes.

Here's the gist of this one: your body partly clears brexpiprazole (Rexulti) using a liver enzyme called CYP2D6. Some lab studies hint that melatonin might slow that enzyme down a little. If that happened, brexpiprazole could build up slightly, and in theory you might notice more of its usual effects, like daytime drowsiness, dizziness, restlessness, or a bit of weight gain.
The honest part: this is a very weak, theoretical concern based only on test-tube research, not on real patients. It's rated minor for a reason. For most people taking normal sleep doses of melatonin, it's unlikely to cause a real problem. Still, since both can make you sleepy, it's worth a quick word with your pharmacist or doctor before combining them, especially if you feel extra groggy.

If that happened in your body, brexpiprazole could clear a little slower and build up slightly, which might mean a bit more of its usual effects like drowsiness, restlessness, dizziness, or weight changes.
I want to keep this in perspective, though. This is a theoretical, minor concern based only on test-tube research, not real patient reports, so for most people it's unlikely to cause trouble. Still, if you notice extra sleepiness or feeling "off" after adding melatonin, mention it to your pharmacist or prescriber before changing anything.

What Is Brexpiprazole Used For?
Brand name: Rexulti
Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.
Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
