Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for Rexulti (brexpiprazole). Your body clears this medicine partly through a liver enzyme called CYP3A4. Goldthread contains berberine, which lab studies hint might slow that same enzyme down. If that happened, brexpiprazole could clear more slowly and build up in your system, nudging up the chance of side effects like drowsiness, dizziness, restlessness, weight gain, or feeling lightheaded when you stand.

I want to be honest about the evidence though: this is purely theoretical, based on how berberine behaves in the lab, and it hasn't actually been reported in people. So I wouldn't panic. Still, since brexpiprazole sticks around in the body a long time, it's worth a quick chat with your pharmacist or doctor before adding goldthread.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
