Why this applies to Modafinil

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for Modafinil: your liver uses an enzyme called CYP3A4 as one of the ways it clears this medicine out. The berberine in goldthread might gently slow that enzyme down, at least based on lab work. If that happened, modafinil could hang around a bit longer than usual, which in theory could nudge up its usual effects like trouble sleeping at night, headache, jitteriness, a faster heartbeat, or feeling anxious.

The important part: this is theoretical. It has never actually been reported in real people, and modafinil is not a prodrug, so it isn't switched on by this enzyme. I wouldn't lose sleep over it, but it's worth a quick mention to your pharmacist or doctor before pairing the two, especially with concentrated berberine supplements.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil helps adults stay awake when they have excessive sleepiness linked to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD). Both Provigil and generic modafinil tablets are labeled for these three uses.

In OSA, it treats the sleepiness only. It does not treat the airway blockage itself. If CPAP (a breathing machine) is your treatment of choice, a full effort to use it should come first and continue during treatment.
