Why this applies to Cyclosporine

Cyclosporine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the situation with goldthread and your cyclosporine. The berberine in goldthread may slow down an enzyme in your liver called CYP3A4, and that's one of the main enzymes your body uses to clear cyclosporine. If that enzyme gets blocked, cyclosporine can build up higher than expected in your blood.

That matters because cyclosporine is a narrow window drug, meaning small changes in its level can cause problems. Too much can stress your kidneys, raise your blood pressure, cause tremor or headache, or push your potassium up. The catch is this is mostly a theory based on lab work, not something proven in people taking goldthread. Still, with a drug this finicky, I'd play it safe. Please check with your transplant team or pharmacist before adding goldthread, since they can watch your cyclosporine levels.

Here's the concern with pairing goldthread and cyclosporine. Goldthread is rich in berberine, which may slow down a liver enzyme called CYP3A4. That same enzyme is what your body uses to clear cyclosporine. If it gets blocked, cyclosporine can build up in your blood and reach higher levels than your doctor intended.

Cyclosporine is a tricky medicine where small changes really matter, so higher levels could mean more side effects like kidney strain, high blood pressure, tremors, or headaches. To be fair, this is based on how the herb behaves in the lab, not on strong human studies. Still, because cyclosporine is so finely dosed, I'd talk with your transplant team or pharmacist before adding goldthread or berberine.

Cyclosporine is prescribed to help prevent the body from rejecting a transplanted kidney, liver, or heart. It is often used alongside other immunosuppressive medicines as part of a full transplant care plan.

It is also used for adults with severe, active rheumatoid arthritis that hasn't responded well enough to methotrexate, and for adults with severe plaque psoriasis who haven't had success with other standard treatments.
