Why this applies to Rifampin

Rifampin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the gist for rifampin. Goldthread contains berberine, which in very early lab work might gently block a liver enzyme called CYP3A4. Since rifampin is partly handled by that enzyme, the theory is that goldthread could let a bit more rifampin linger in your system, which might nudge up its effects or side effects like nausea, stomach upset, or liver irritation.

That said, this is purely theoretical and has never actually been seen in real patients. There's also some irony here, since rifampin itself is a powerful enzyme booster that can weaken many other medicines. I wouldn't lose sleep over it, but because TB treatment matters so much, please tell your pharmacist or doctor before adding goldthread.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis (TB). It is almost always used in combination with other antibiotics — your doctor will build a multi-drug regimen tailored to your infection. Treatment usually runs for at least 6 months total.

Rifampin is also used to clear Neisseria meningitidis bacteria from the nose and throat in people who carry the germ without being sick themselves. It is not used to treat active meningococcal disease. The intravenous (IV) forms — Rifadin IV, Rifadin IV NOVAPLUS, and the injectable form of Rifampin — are specifically indicated when a patient cannot take the drug by mouth.
