Why this applies to Miconazole

Miconazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the good news first: most folks use miconazole as a cream, suppository, or powder for yeast infections (think Monistat or athlete's foot products). When it's used that way, very little gets into your bloodstream, so goldthread is very unlikely to cause a problem. The concern on file is about berberine, the active compound in goldthread, possibly slowing down a liver enzyme called CYP3A4. If that happens, drugs cleared by that enzyme could build up.

This is only a theoretical, lab-based idea and hasn't been seen in real people. Interestingly, miconazole itself blocks the same enzyme. If you ever take miconazole by mouth or in larger amounts, it's worth a quick chat with your pharmacist before adding goldthread.
