Why this applies to Clomipramine

Clomipramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Goldthread interacts with.

There's very preliminary evidence that berberine, a constituent of goldthread, might inhibit cytochrome P450 3A4 (CYP3A4) enzyme. So far, this interaction has not been reported in humans. However, watch for an increase in the levels of drugs metabolized by CYP3A4 in patients taking goldthread. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), clarithromycin (Biaxin), indinavir (Crixivan), sildenafil (Viagra), triazolam (Halcion), and numerous others. Use goldthread cautiously or avoid in patients taking these drugs.

Here's the basic idea with goldthread and your Clomipramine. Goldthread contains berberine, and lab work hints that berberine may slow down a liver enzyme called CYP3A4. Your body uses that enzyme as one of the routes to clear clomipramine. If that route is partly blocked, clomipramine could build up a bit more than usual.

What might that feel like? More of clomipramine's usual side effects, things like dry mouth, drowsiness, constipation, blurred vision, dizziness, or a faster heartbeat. I want to be honest though: this is theoretical, based on how berberine behaves in the lab, not from real patient reports. Still, since clomipramine has a fairly narrow safety window, it's worth a quick chat with your pharmacist or doctor before adding goldthread. Please don't change your dose on your own.

Clomipramine — sold as Anafranil and under the generic name Clomipramine Hydrochloride — is approved to treat obsessions and compulsions in people with Obsessive-Compulsive Disorder (OCD). This includes adults and children/adolescents ages 10 to 17.

To qualify for treatment, the obsessions or compulsions must cause significant distress, take up a lot of time, or noticeably interfere with work, school, or social life.
