Theoretically, Eurycoma longifolia might increase levels CYP1A2 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP1A2 enzymes. This effect has not been reported in humans.

Here's the gist for tongkat ali and clomipramine. Your body partly relies on a liver enzyme called CYP1A2 to break clomipramine down. In lab dish studies, this herb weakly slowed that same enzyme. If that happened in real life, clomipramine could clear a little more slowly and build up, which might mean stronger side effects like dry mouth, constipation, drowsiness, dizziness, a racing heartbeat, or trouble urinating.

Now the reassuring part: this is only a theoretical concern based on test-tube work, and it has never been reported in actual people. The blocking effect was also weak. Still, since clomipramine is a medication where dose matters, it's worth a quick chat with your pharmacist or doctor before adding this supplement.

Here's the basic idea: your body uses a liver enzyme called CYP2C19 to help break down clomipramine. In lab dish (test tube) studies, tongkat ali appears to weakly slow that enzyme down. If that happened in people, clomipramine could clear a little more slowly and drift higher in your blood. Clomipramine is an active drug (not a prodrug), so more of it means stronger effects, things like dry mouth, constipation, drowsiness, blurred vision, dizziness, or a faster heartbeat.

That said, this is purely theoretical and has never been shown in real people, and the effect looked weak. So I wouldn't panic. Still, since clomipramine needs careful dosing, it's worth a quick chat with your pharmacist or doctor before adding this supplement.
