Why this applies to Vortioxetine

Vortioxetine falls under CYTOCHROME P450 2A6 (CYP2A6) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2A6 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2A6 enzymes. This effect has not been reported in humans.

Here's the short version: this is a very theoretical concern, and I wouldn't lose sleep over it. The worry is that tongkat ali might slightly block an enzyme called CYP2A6, which could let drugs cleared by that enzyme build up a bit. With Trintellix (vortioxetine), that would in theory mean slightly higher levels and a higher chance of common side effects like nausea, headache, dizziness, or vomiting. Vortioxetine is an active drug, not a prodrug, so more in the blood means more effect, not less.

That said, this is based only on lab dish studies, never shown in real people, and vortioxetine is mostly handled by a different enzyme (CYP2D6) anyway. I'd just mention it to your pharmacist or doctor before adding the supplement, and let them know if you feel off.
