Why this applies to Cyclophosphamide

Cyclophosphamide falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Eurycoma Longifolia interacts with.

Theoretically, Eurycoma longifolia might increase levels of CYP2C19 substrates.
In vitro research suggests that methanolic Eurycoma longifolia root extract weakly inhibits CYP2C19 enzymes. This effect has not been reported in humans.

Here's the wrinkle worth knowing: cyclophosphamide is a prodrug. It arrives inactive and your liver enzymes, including CYP2C19, switch it on into the form that actually fights cancer. So if Eurycoma longifolia were to slow that enzyme down, the concern would be a bit less of the active drug being made, not more.

That said, this is a very weak, theoretical concern. The only signal comes from lab dish (in vitro) studies showing mild CYP2C19 blocking, and it has never been seen in real people. Still, because cyclophosphamide is serious chemotherapy where dosing is carefully balanced, don't add tongkat ali without first checking with your oncology team or pharmacist so nothing affects how well your treatment works.

About Cyclophosphamide

What Is Cyclophosphamide Used For?

Brand names: Cytoxan, Neosar

Cyclophosphamide is used to treat many types of cancer in adults and children, including malignant lymphomas (such as Hodgkin's disease and Burkitt's lymphoma), multiple myeloma, several forms of leukemia, breast cancer, ovarian cancer, neuroblastoma, retinoblastoma, and advanced mycosis fungoides (a type of skin lymphoma). It is often given alongside other chemotherapy drugs rather than on its own.

Oral and injectable powder formulations (but not the Cyclophosphamide Injection solution concentrate) are also approved for minimal change nephrotic syndrome — a kidney condition — in children who have not responded well to, or cannot tolerate, steroid therapy. The injectable solution concentrate is approved for adult patients only due to its alcohol content, and is not approved for use in children.
